If the form is an odd number of pages and the range of patterns printed from one document (.ps file) extend over multiple form instances. The printer has no way to know when a new instance should start and end for a new page when printing duplex from one document. Here are few options if you wish to continue printing duplex:

Make your page count an even number.
Create and print separate PostScript (.ps) files for each form instance.
